This form is to be used to request new instructional technology software to be used in District 215. In accordance with state law and District policy 7:345, only software that has been approved by the Superintendent or designee may be utilized in District 215. Any new software implemented in District 215 must have a demonstrated curricular need and must comply with applicable state law and District policy. Completing the form below begins the review process. Provide detailed answers to the questions below. Upon submission, the Department of Teaching and Learning will review your request. If approved, the Technology department will further evaluate.
This review process will take time and may vary in length depending on factors including the completeness of your request and responsiveness of the vendor. You will receive updates as your request is processed.
