  • New Student Enrollment Form

  • All students new to the district (except for students coming from one of our feeder schools) must enroll with the registrar.

  • New Enrollment is Defined as:

    • Student was enrolled in a high school outside of the defined District 215 boundaries.
    • Student was enrolled in a high school outside of the State of Illinois.
    • Student was not enrolled at Burnham, Lincoln, Schrum, or Wentworth JRHS last school year. Student received 8th grade certificate of completion (Diploma).

    1. Never attended Thornton Fractional North or South High School
    2. New to an Illinois School
    3. Transferring/entering from a Jr high or high school outside of District 215 boundries

  • Student Primary Address

    The student address information you provide will be electronically verified by District 215 to ensure that the student lives within the boundaries of District 215. If your address is unable to be verified, you may be asked to submit documentation to prove your residence. It is violation of Illinois Law to knowingly provide false address information for the purposes of school residency, anyone violating this law may be prosected.
